n. As a Family Service Support Person, you will play a vital role in providing crucial support and guidance to individuals and families navigating various challenges. You will utilize your established skills in eligibility determination, case management, and understanding of government assistance programs to help clients access the resources they need to achieve stability and well-being. This is a rewarding opportunity for a motivated individual who is adept at building rapport, problem-solving, and advocating for vulnerable populations.
Responsibilities:
Conduct thorough client intakes and assessments to identify needs and develop individualized service plans.
Accurately determine client eligibility for various programs and services based on established guidelines and regulations.
Proficiently calculate client qualification for assistance programs, ensuring compliance and accuracy.
Provide comprehensive case management services, including connecting clients with relevant community resources, referrals, and support networks.
Maintain detailed and organized client records, ensuring confidentiality and adherence to agency policies.
Collaborate effectively with other agency staff, community partners, and government agencies to provide holistic support.
Assist clients in navigating complex systems, such as housing, welfare, and temporary assistance programs.
Advocate on behalf of clients to ensure their needs are met and their voices are heard.
Monitor client progress and adjust service plans as needed.
Participate in team meetings and professional development opportunities.
Working hours: 8:00 AM - 5:00 PM

Ability to build rapport and establish trusting relationships with diverse populations.
Organizational and time management skills with the ability to prioritize tasks effectively.
Proficiency in record-keeping and data management.

Education:
High School
Experience:
1-4 years
Qualifications:
Proven experience (typically 2-5 years) in a direct service role involving eligibility determination and case management.
Demonstrated ability to understand and apply complex eligibility criteria and accurately calculate client qualifications for various assistance programs.
Experience working within local and/or state government systems processing housing, welfare, or temporary assistance programs is highly preferred.
A background in social work or a related human service field is desired.
Understanding of community resources and social service systems
A Bachelor's degree in Social Work, Human Services, or a related field is preferred. Equivalent experience may be considered.
