- Collects information regarding potential childrens long-term support program applicants; makes functional screen visits to meet with the child and parents to assess childs level of functioning; verifies required documents; assists families with completion of applications and recertification paperwork;answers familys questions about programs the family wishes to apply for and refers families to community agencies, when appropriate.- Completes a web-based Children?s Long-Term Support Functional Screen for each child, following established protocol and deadlines.- Completes Medicaid request for health care using CARES Worker Web system- Assess and confirm all Medicaid Eligibility requirements- Collaborates with staff at the Department of Health Services to collect information necessary for applications.- Ensures clear, timely communications and reporting.- Maintains orderly, current records of child /family information, contacts, and monthly statistics.- Participates in assigned committees/task forces/special projects and attends in-services and educational programs to maintain and enhance professional expertise.- Participates in the meetings to enhance and improve practice through shared practice, learning and training.- Makes referrals to alternative programs when appropriate

- Understands the needs of the neonatal, infant, toddler, pre-school, school-age, adolescent patient in regards to their growth and development process- Excellent communication skills (verbal and written) and the ability to relate to people with varying socioeconomic classes with differing lifestyles and/or cultural values.- Knowledge of support and service resources for the designated service region or demonstrates the ability to develop that knowledge.- Possess strong attention to detail, timelines and established practice standards.- Home visiting experience is strongly preferred.- Experience with the Children's Long-Term Support Functional Screen preferred.- Experience with CARES Worker Web preferred.- Flexibility within schedule to accommodate parents; availability for functional screen visits.- Excellent organizational skills and ability to work autonomously.- Demonstrated computer experience required.- Ability to present program material in a variety of settings.- Requires a valid driver's license, acceptable driving record and the ability to be insured.
- Bachelors degree in a human services related field and at least two years of experience working with children who have disabilities and/or complex medical needs, their families and support systems or an Associate Degree in a human services related field and five years of relevant experience.- Ability to evaluate and analyze complex needs of children including medical, emotional, developmental and financial and complete required state protocols.- Telecommuting for day-to-day office work- Teleconferencing for functional screen visits- Quarterly travel to Madison for in person meetings- Potential, but infrequent, travel statewide for functional screen visits
