The Eligibility Specialist is responsible for intake and eligibility determination for the Katie Beckett Medicaid Program, including conducting Children's Long-Term Functional Screens (CLTS FS) and reviewing medical records. This role involves assisting families through the application process, ensuring timely and accurate eligibility assessments, and referring families to other support programs as needed. The position requires excellent communication skills, attention to detail, and the ability to work independently.
Responsibilities:
- Consult with families and staff regarding program eligibility and procedures
- Conduct functional screen visits with children and families to assess eligibility
- Gather and review health records related to program requirements
- Complete web-based Children?s Long-Term Support Functional Screens
- Provide information and referrals to other services and community agencies
- Maintain accurate records and report monthly statistics
- Participate in committees and training opportunities
Qualifications:
- Bachelors degree in a human services field (or Associate degree with 5 years of experience)
- At least two years of experience working with children with disabilities or complex medical needs
- Strong communication, organizational, and analytical skills
- Ability to work independently and manage time effectively
- Experience with virtual intake visits and functional screen assessments preferred
